I am an out of network provider for all insurers with the exception of Blue Cross products. Some insurers provide an out of network benefit which may cover your visit. Please contact your insurer to see if you have an out of network benefit. Most insurers will have a Customer Service number on the back of your insurance card. It is important to ask if you have the benefit as well as your approved number of visits, your co pay and your plan’s deductible.
I accept payment in the form of cash, check or money orders. I am also able to accept all major credit cards and debit cards. In addition, I offer a sliding fee scale and a reduced rate of service for those experiencing financial hardship.
Documentation of your visit and associated costs can be made available for flexible spending accounts or premium deductibles.
Cancellation of your appointment is required at least 24 hours before your visit. Any cancellation without the proper 24 hour notice will be charged to your account. Remember, if you cancel in time, someone else is able to access care and you will not be charged for your visit.
